🤦🏽‍♂️I’m not sure who taught all of us the above statement (yes, I too, once made this erroneous statement). To really understand what is sinus rhythm you have to understand what is happening electrically in the heart. .

✅First things first...where is the sinus node?? The sinus node sits in the high right atrium near the entrance of the SVC. The sinus node is the pacemaker for the heart sending electrically impulses when stimulated (and inhibited) to do so by the brain (autonomic nervous system). When the sinus node fires where does the impulse go to next?? ↘️The impulse travels down specialized tracts in the right atrium and across the left atrium (via Bachman’s bundle) to depolarize the atrium (P wave on EKG). .

👁So when you look at an EKG you have to comprehend electrical vectors. If the sinus node is firing from high right atrium to the crux of the heart (AV node) the wave of depolarization is downward and slightly toward the left (southeast direction↘️). Next you have to understand where the leads sit overlaying the heart. The best place to look for sinus activity is leads I, II, and AVR. Look at the video above and see why! These leads give you the direction of what is happening in the right atrium. If you have sinus rhythm...that means the sinus node (high right atrium) is firing in its normal position down towards the AV node (up right leads I and II (or often easier to remember inferior leads) and inverted in AVR).

💡Many conditions can have sinus rhythm without having “a p before every QRS.” An example is complete heart block that has upright P waves in leads I and II however AV dissociation (P’s and QRS doing their own thing). We still say there is sinus rhythm but there definitely is NOT a P before every QRS! .

💣If you want to impress on your cardiology rotations start with this! It’s a cardiologists “nails on a chalkboard” to hear the dreaded statement “P BEFORE EVERY QRS” END THE MADNESS!!!! Learn the P WAVE AXIS and VECTOR!!!! .

📚One of the most tested areas on echo boards is knowing which wall segments correlate to which coronary artery. Additionally in clinical practice (especially with tools like @butterflyiq) you have the ability to assess for wall motion abnormalities before taking patients to the cath lab, and thus, have a suspicion for what coronary artery can really help guide your approach. .

🎥The best view for assessing wall motion/coronary distribution (in my opinion) is the parasternal short axis (PSAX) through the mitral valve to apex. This view is easily attainable by placing the probe at the 3rd-4th intercostal space and having the marker face the left shoulder. Here the ventricular will look like a donut 🍩 , with the walls on the left being septal (inferior and anterior septal), the top being anterior, the right being lateral (anteriolateral and inferiolateral) and the bottom being inferior. .

📝Knowing which arteries correlate to which EKG leads and which walls is a foundation for general cardiology and I hope this illustration comes in handy! .

🔝🎦The above video is an example of one of the physical exam findings to look for in chronic aortic insufficiency. This finding is called Quinke’s pulses. It is preformed by pressing and holding the nail beds of the patient and assessing for capillary refill. In chronic AI there is a large systolic stroke volume (due to regurgitation during diastole into the LV), wide pulse pressure (the difference between the systolic and diastolic pressure), and a rapid fall in Arterial blood pressure (due to regurgitation) leading to the nail beds and other vascular beds demonstrating this rapid fill/empty pattern. .

♥️Corrigan's pulse: prominent pulsations of the carotid arteries

♥️Bisferiens pulse: double systolic arterial impulse - the so-called twice beating heart

  • Wishing everyone a very Happy Easter!🐰🥚🎁. My #happyeaster story with a #tip for apple watch users:
This is what I found inside my easter egg, the Apple Watch Series4😄. This time I preferred the gold aluminium case instead of the boring space grey on my series 2 and also the most common colour which everyone seems to wear. I like its bigger screen (44mm) and the extra real estate which comes with it. The unboxing was a surprise as the box and packing inside was just amazing this time around with the watch itself packed in a beautiful pink colour jacket. I got the classy leather hybrid sports band too as the pink sports band which comes inside the box is very light in color and gets dirty only after a couple of workouts. I love the apple watch as it motivates me to be healthy on a daily basis and even looks classy on your wrist as an evening wear. It was difficult to get through my workouts even for a day without it. The S2 has been my constant workout companion since the last couple of years now. The new S4 is a gift thrust upon me this Easter (not that I'm repenting it one bit; a blessing in disguise😇) after my S2 gradually died down on me during my last couple of swims. But wasn't it supposed to be water proof and meant for swimming? I took it to the Apple service centre and they told me that the top glass screen had a very big scratch which punctured its water resistance properties and so the water seeped in and damaged its display and some other internals. Note to myself and all those reading this: If you use an apple watch then better use a good screen protector to safeguard against all such accidental scratches some of which could be fatal for it and spell its doom!⌚️😎

É sempre bom ter além do livro outras formas de estudo, pra mim funciona bem misturar o que foi anotado em sala + slides + livros, usando a técnica pomodoro (ainda vou fazer um post só da técnica 😉)
