Never forget how far you’ve come ❤️ Something I continue to struggle with in residency is always feeling inadequate at my job and like I’ll never know enough. But then I remember how far I’ve come in less than 5 years and know that these next 5 spent learning the field of general surgery will fly by. Any of you continue to feel inadequate and like there’s hopelessly too much information to ever learn?? It gets better; I promise. 😊

  • {9.12.19🎄} Zdjęcie trochę sentymentalne, bo oficjalnie przygodę z OUNem zakończyłam wynikami koło 80% zarówno ze szpilek jak i z kolokwium teoretycznego 😅 Nie był to najgorszy dział, dało się go przeżyć i nawet czasem coś zrozumiałam z tej czynnościowej (chociaż wciąż za mało, żeby umieć porządnie odpowiedzieć na niektóre pytania ). Grunt, że to już za mną 🥳 Dzisiaj za to były pierwsze zajęcia z GiSu (dla niewtajemniczonych: głowa i szyja) i mimo, że to już trzeci miesiąc studiów to poczułam się jakbym była tam pierwszy raz, ze względu na obecność całych zwłok w prosektorium (wcześniej oglądaliśmy tylko wypreparowane fragmenty mózgowia) 😳 Jakie to uczucie? Powiedziałabym, że dziwne i bardzo śmierdzące (przez formalinę oczywiście), przebywanie tam przez te 2h to troszkę droga przez mękę, ale pewnie przywyknę 🤷🏻‍♀️ Jestem pełna podziwu dla ludzi, którzy decydują się oddać po śmierci swoje ciało, żeby takie studenty jak my mieli się na czym uczyć, naprawdę niesamowita sprawa😇 Na dniach postaram się napisać wam co nieco o psychologii medycznej, bo udało mi się zaliczyć kolokwium na całkiem ładny wynik, także oficjalnie mam ją z głowy i mogę coś się wypowiedzieć 🥰 No i nie byłabym sobą gdybym nie wspomniała, że już czekam na święta,  miłoby było trochę w końcu odsapnąć 😴 Uciekam jeszcze na chwile do anatomii, tętnice szyjne czekają ✨❤️ { #studygram #studygrampl #studygrampolska #medstagram #medgram #medstudent #medicine #medycyna #lekarski #futuredoctor #anatomy #anatomia #histologia #histology #studyhard #study #nauka #studying #notatki #notes #notatkidomatury #notatkibiologia #matura2020 }

  • It’s finals time!! ⏱Studying for finals can be stressful and overwhelming but we have some tips that we hope can help make it manageable! 1- Plan ahead!!! Make a schedule with the dates of all your exams and how you want to prioritize study time based on the amount of material and how close the test is 🗓 2- If you have access to the exams/homework you have taken throughout the semester make sure to go through and redo them. This will help identify areas you may have forgotten and sometimes professors put similar problems on the final exam! ✏️ 3- Study with a friend and quiz yourself! This gives you a chance to destress by having the company of someone you enjoy being with . Plus, active learning is one of the best ways to successfully retain information. Who’s that special person in your life that gets to share quality study time with you?👬👫👭 Tag them so we can appreciate them. Happy Studying 🤓⭐️📚📕📗📙📘

  • Do you have a mentor? What is the point of a mentor? ⬇️⬇️
👨‍⚕️
Mentors are like the Sriracha to your eggs. They help you reach your full potential and help boost all the right qualities to make you shine. But seriously, mentors support you in different areas of your life at different times. I was once told to “build your board of directors” the mentors who will help you to succeed and I couldn’t agree more!

I like to think that you borrow a small piece from each mentor as you grow into a young physician.

This particular attending, Dr. Kalla is one of many amazing mentors and friends I have connected with through medical school. He is an anesthesiologist and a phenomenal teacher. I always give full credit for my intubation and central line skills to him!

I think a big point behind sharing this is to open your mind about mentorship. You can have a surgical mentor, a family physician, an OB, maybe even a lawyer or a CEO. The point is everyone has something to teach you, keep your mind open and you just might be surprised!

  • I got to be involved in 6 deliveries last night! 7 babies total because twins! 😍 I’ve never been this tired. 😴

I don’t think I sat for more than 5 minutes at a time. I’m exhausted, but it was easily the best shift of my OB/Gyn rotation so far. I came home, slept, and even got a few hours of studying in. 🙃

Now I’m off for another night of hanging out with the midwives, nurses, doulas, doctors, and patients! 💕

  • Ganglion cyst of the foot being drained out!! A ganglion cyst is a sac filled with a jellylike fluid that originates from a tendon sheath or joint capsule. The word “ganglion” means knot and is used to describe the knot-like mass or lump that forms below the surface of the skin. 
Ganglion cysts are among the most common benign soft-tissue masses.
They are not harmful or cancerous, although they may cause discomfort and tingling, burning, numbness, aching, or pain if they are on or near a nerve.
Although they most often occur on the wrist, they also frequently develop on the foot—usually on the top, but elsewhere as well. Ganglion cysts vary in size, may get smaller and larger and may even disappear completely, only to return later.

This technique involves draining the fluid and then injecting a steroid medication into the mass. More than one session may be needed. Although this approach is successful in some cases, in many others, the cyst returns. Needle aspiration is also used as a treatment.

Case of a truck driver who degloved his ring finger while getting out of his truck. While climbing out, he jumped the last step of the truck, not realizing the ring got hooked on the door. The result was a degloved ring finger with the avulsed tendon still attached to the tip.

A degloving injury is a type of avulsion in which an extensive section of skin is completely torn off the underlying tissue, severing its blood supply. The entire musculo-skeletal unit of the finger is intact, and the patient can often move the parts of his naked hand quite normally. It is named by analogy to the process of removing a glove. Degloving injuries invariably require major surgical interventions.

Treatment options include replantation or revascularization of the degloved skins, or when these are not possible, skin grafts or skin flaps. While the preservation of the extremities and limbs is normally preferred, in some cases, especially digits, amputations may be advised or required.

Ultrasound of the scrotum and testicles demonstrated a large left hernia containing anechoic fluid and omental fat.  CT of the abdomen and pelvis demonstrated multiple soft tissue nodules in the large left hernia. In conjunction with the finding of multiple splenules in the splenic fossa, a diagnosis of splenosis was suspected. The patient gave a history of prior trauma. A heat damaged red blood cell study demonstrated multiple foci of increased radiotracer accumulation in the abdomen and left scrotum consistent with a diagnosis of splenosis.

Splenosis results from heterotopic auto transplantation of splenic tissue as a result of trauma or surgical intervention. It is a benign condition often discovered as an incidental finding on imaging and can pose a diagnostic challenge. Occasionally it may present with symptoms related to obstruction, compression or gastrointestinal bleeding. Accessory spleens are congenital , confined to locations near the splenopancreatic or gastropancreatic ligament. Splenosis can be intraperitoneal or extraperitoneal and found anywhere.

Tc-99m sulfur colloid has been used to diagnose spleen based on its ability to localize in reticuloendothelial tissue. Heat damaged rbc study is however more sensitive and specific for the diagnosis of splenosis.

Oggi ho fatto la terza e ultima esperienza di laboratorio ma devo dire che è stata la più stimolante in assoluto!! 😜 Abbiamo creato delle soluzioni saline a diversa concentrazione e aggiunto 4 gocce di globuli rossi in ognuna. Le provette si sono colorate di rosso e il soluto si è diffuso... (Già questo è stato bello😂)
Abbiamo messo a centrifugare per 5 minuti e contemporaneamente siamo riuscite a vedere le cellule al microscopio anche se c'è stata parecchia delusione nel vederle incolore (➡️ books VS real life).
Per finire abbiamo misurato lo spettro di assorbimento delle 9 soluzioni, dall' ipertonica all' ipotonica... E abbiamo visto una cosa incredibile!😱 I globuli rossi in acqua (sol. Ipotonica) subiscono emolisi in modo minore rispetto ad una soluzione di acqua con pochissimo sale disciolto (sol. Ipotonica) e nessuno né sa ancora il perché... Magari un giorno sarete proprio voi a scoprilo!!

🇬🇧 Today I added some Xmas lights on my desk and I didn't expect it would turn out to be so cute? Also I did the longest study with me on Youtube and it went so well, thank you to everyone who joined me! 🇮🇹 Mi sono resa conto poco fa che ho contato male i giorni che mi mancano dall'esame e ho un giorno in più del previsto! 
Ho comunque un programma super compatto, voglio finire di ripetere 2-3 prima dell'esame così gli ultimi giorni li dedico al mega ripassone dove mi guardo tutto il programma 10 volte 😰

Anche se sto studiando pure dopo cena mi lascio gli argomenti più leggeri o comunque qualcosa che devo solo leggere e non ripetere, cerco di "assecondare" i miei picchi di attenzione con argomenti più o meno faticosi. Di solito il massimo lo raggiungo subito dopo pranzo (dalle 14:30 alle 16 studio filata senza distrazioni). Qual è il vostro orario migliore per studiare?

💊 Much of what we do is medication management. Many people here have chronic mental illnesses that were not diagnosed here and a med list that they sometimes don’t follow. Many times initial management includes restarting them on their home meds or beginning trials of different meds to see if those work better. Some psychiatric meds take 4-6 weeks to kick in (I’m looking at you, SSRIs!) or it takes time to titrate someone up to a therapeutic dose. But slow and steady wins the race, and we find the right regimen for our patients. Or we send them to ECT. More on that Wednesday!🤗

👩🏻‍⚕️As primeiras duas semanas de psiquiatria no hospital acabaram e percebi que é mais sobre “devagar se vai ao longo.”
•
Muito o que fazemos é dirigir medicamentos. Muitas pessoas aqui têm doenças mentais crônicas que não foram diagnosticadas aqui e uma lista de medicamentos que às vezes não seguem. Então a gestão inicial inclui começar de novo os medicamentos de casa ou fazer testes de medicamentos diferentes para ver se funcionam melhor. Alguns medicamentos levam ~4-6 semanas para fazer efeito (estou olhando pra vocês, ISRS!) ou leva tempo para chegar à uma dose terapêutica. Mas devagar se vai ao longo, e encontramos um regime para nossos pacientes. Ou enviamos eles para ECT. Mais sobre isso a quinta-feira!🤗

Mucho que hacemos es manejar medicamentos. Muchas personas aquí tienen enfermedades mentales crónicas que no fueron diagnosticadas aquí y tienen una lista de medicamentos que a veces no siguen. Entonces el tratamiento inicial incluye comenzar los medicamentos de casa de nuevo o comenzar pruebas de medicamentos diferentes para ver si funcionan mejor. Algunos medicamentos llevan tiempo a tomar  efecto o lleva tiempo llegar a una dosis terapéutica. Pero lento pero seguro encontramos un régimen para nuestros pacientes. O les mandamos a hacer TEC.   • My journey so far:

I started med school extremely excited. I was so excited that I used to read books about other doctors experiences. I would soak up anything and everything related to medicine. My seniors used to find me a bit too enthusiastic. While I wondered why they were not enthusiastic enough.
⠀
Then came 3rd year, the hardest year at my university. It broke me, not only mentally but emotionally. That enthusiasm that I had, gone. The desire to learn, gone. My mental health, in shambles.
⠀
I started to deal with mental health issues that I never had before. Anxiety, social withdrawl, breakdowns for no apparent reason and it was all due to the extreme pressure I was under. I had to get my stress under control. Thankfully I found weight lifting and it helped tremendously, until I got injured (but that’s a story for another day).
⠀
The months went by and I was still burned out. I found no joy in medicine anymore, just a mere obligation. Deep down I knew it was my passion and what I wanted to spend my life doing but I was just so mentally exhausted. So the days went by and I kept trudging along.
⠀
Then I met my first patient, he was a boy about the age of 10. I got to do my first physical examination on him and I was soaring. It all came back full circle, that boy alone made all the suffering worth it.
⠀
This journey has beat me down so many times, but alhamdullilah I managed to get back up with Allah’s help. I have truly grown so much because of these experiences and I’m honestly thankful.
⠀
I salute every medical student and every doctor because this profession is tough. It will take a toll on you mentllay, physically and emotionally but the patients make it worthwhile.

    I started med school extremely excited. I was so excited that I used to read books about other doctors experiences. I would soak up anything and everything related to medicine. My seniors used to find me a bit too enthusiastic. While I wondered why they were not enthusiastic enough.

    Then came 3rd year, the hardest year at my university. It broke me, not only mentally but emotionally. That enthusiasm that I had, gone. The desire to learn, gone. My mental health, in shambles.

    I started to deal with mental health issues that I never had before. Anxiety, social withdrawl, breakdowns for no apparent reason and it was all due to the extreme pressure I was under. I had to get my stress under control. Thankfully I found weight lifting and it helped tremendously, until I got injured (but that’s a story for another day).

    The months went by and I was still burned out. I found no joy in medicine anymore, just a mere obligation. Deep down I knew it was my passion and what I wanted to spend my life doing but I was just so mentally exhausted. So the days went by and I kept trudging along.

    Then I met my first patient, he was a boy about the age of 10. I got to do my first physical examination on him and I was soaring. It all came back full circle, that boy alone made all the suffering worth it.

    This journey has beat me down so many times, but alhamdullilah I managed to get back up with Allah’s help. I have truly grown so much because of these experiences and I’m honestly thankful.

    I salute every medical student and every doctor because this profession is tough. It will take a toll on you mentllay, physically and emotionally but the patients make it worthwhile.

  • Tematická vánoční fotka 😂🎄
Od minulého pátku mám “pohodu”, jen dnes test z fyziologie a ještě bude angličtina. Opravdu si troufám říct, že druhák je v pohodě. Možná to není druhák, ale já a všichni kolem 🤓. Takže  prváci nebojte. SKONČÍ TO, aspoň na chvíli (😀🙈). Za měsíc a kousek už zkouška, tak přemýšlím, zda je čas se na ten předmět poprvé podívat aneb genetika 🙅🏽‍♀️ láska! 
Nicméně lehce závidím přátelům z jiných VŠ, kde si zkoušky mohou odbýt už před Vánoci a mít pak v lednu klid, my teď sice máme pohodu ale v lednu umřeme a kdyby byla možnost, tak nějaký ten započtík z fyzio nebo genetiky bych si klidně dala teď, než všechno ten 14. Týden. ( proč nedat VŠECHNY zápočty v jeden týden 🥰) Už ve vás klíčí vánoční nálada ?

  • Cerebral cavernomas are benign, low flow vascular hamartomas consisting of a conglomeration of blood vessels lacking smooth muscles, elastic fibers, feeding arteries and draining veins. They don't contain neural tissue. Most are supratentorial, but they can also occur in the brainstem, basal ganglia and rarely in the spinal cord. Can be asymptomatic (50%) but, when symptomatic, they manifest themself with seizures (60% of which 40% are medically refractory) or progressive neurologic deficits (50%). Hemorrhages are less common (20%) and usually are small and recurrent, which is why cavernomas are surrounded by a yellow halo of hemosiderin. They can grow, regress or remain stable.

TC is neither sensitive nor specific. RM is the gold standard exam to diagnose cavernomas ("popcorn" pattern). AGF is not necessary in classically appearing cases.

The treatment depends on location, lesion's characteristics and symptoms. Options concern: follow-up, surgical excision, stereotactic radiosurgery. If the lesion is accessible and symptomatic, surgery should be preferred. In brainstem cavernomas that have not bled, surgery is not recommended.

The greatest difficulty in small cavernomas (like this case) is to find the lesion. Stereotactic localization or intraoperative ultrasound may be particularly helpful. Since many difficulties can complicate the localization of the cavernoma with the neuronavigator, the neurosurgeon must be able to use intraoperative ultrasound: the surgery consists in removing the cavernoma following a catheter (video 2) being inserted in the lesion by means of an ultrasound guide (video 1). When completely removed, the risk of further growth or hemorrhages is eliminated.

A 3 months control RM is recommended.

  • HOSTEL SERIES (1/3) 
Have you always wanted to run away from the “madness” of home or maybe just want to be on your own and live life the way you wanted to? Well, I was never one of those but living in the hostel has taught me more than i have learnt in the classroom and there are some things only living away from home can teach you.
Hostel life is meant to be a lot of fun and you live it the right way, you will cherish those memories for a lifetime. Having lived all my life in at home, moving to a hostel taught me lessons in life that books can’t teach you.
The good – Hostels are often like families where each one knows the other like the back of their hand.Its a huge group of young energetic youth waiting to discover the joys of freedom knowing fully well they aren’t accountable for their actions(not right away at least). Eating meals with friends,always having the company you want considering you have a battalion to choose from.
The combine studies! I owe half my marks to roommate and friends.Every hosteler  irrespective of his or her experience will remember the combined study sessions or at least the good times spent in the name of study ! I’ve had friends get high, eat , sleep, talk,scream, yell, and go for long walks in the middle of the night either to unwind  or simply because we gave up on the hopes of passing and still managed pretty decent grades. The good thing about hostels is that you have to live with all kinds of people and if you learn to make the right choices, you will be respected for who you are and will share bonds that will last a lifetime. Loud music, hearty laughs, heated arguments followed by  endless bonding are part of just another average day in life of a hosteler. .
