  • Hypothyroidism: overwhelming fatigue, weight gain, cold hands and feet, brain fog, dry skin and thinning g hair... In my practice I’ve seen a number of #hypothyroid patients (some under 20 yoa!) who were either not diagnosed initially or continued to experience symptoms despite being diagnosed and medicated. As soon as their thyroid issue was properly addressed, they felt SO much better! 
Unfortunately, #thyroid testing and diagnosis are not that straightforward. TSH is usually used for diagnosis and monitoring, which is a brain hormone secreted in response to thyroid activity and supposedly is a reflection of thyroid health. Unfortunately TSH is often not reliable at all. Real life picture is this:

If TSH is high -> thyroid is UNDERACTIVE. 
If TSH is low or normal -> WE JUST DON’T KNOW 
How did we figure out the "normal range" of TSH? A large study ~15 yrs ago determined the "normal range". In order to exclude people with thyroid problems they asked for fam history of thyroid disease and screened for goiter (enlarged thyroid). That's it! 
As a result they figured out the “normal” range for the participants – 0.45 to 4.12 mIU/L. 
Basically, this study likely included so many people with low thyroid function, it's incredible!

More recent studies showed a totally different “normal” range, with the upper limit of normal TSH lowered to 2.5 mIU/L. It was cut by almost half! 
Despite 10 years of research and controversy new values never made it to thyroid testing guidelines. Current "normal" TSH range basically represents the population average - healthy, unhealthy, sick, everyone! 
I use different, OPTIMAL thyroid range supported by most recent research and clinical observations. I noticed  that people feel their best when:

TSH is under 2 mIU/L (compare with conventional under 4.5 mIU/L)

Free T4 is >1.1 NG/DL 
Free T3 is > 3.2 PG/ML 
Most importantly I look at the clinical picture. If a patient displays symptoms of hypothyroidism, I use the above ranges, and then decide if I should treat. No symptoms - no treatment required, unless the numbers are crazy off the chart.

  • What a beautiful two days we've had in the city, and I'm beyond glad that I took some time to finally wind down for a minute and reflect. 🙏🏻

It's been a while since I went out with my camera and captured 📸 some of the natural beauty around here, so we ended up exploring Tommy Thompson Park; somewhere I have yet to visit since moving to Toronto over 20 years ago. 🙈

We heard the incredibly loud chirping 🦆coming from the conservation area when we were out for a walk yesterday, which is why decide we HAD to check out the Ecological Research Area at Tommy Thompson today. I'm very glad that we did. 🙌🏻

What an experience it was to see and hear the hundreds of birds migrating back to the city for the warmer months. 🤩
